In his previous six appearances here at the Madrid Masters, John Isner made the R16 thrice but lost two of them, against David Ferrer in 2014 and Rafael Nadal in 2010.

For the second time in two days, Pablo Cuevas needed three sets to secure his progression (defeated Albert Ramos Vinolas in R2). He will now meet his second American opponent of the week, having previously beaten Jack Sock in the first round.
The Uruguayan is defending his semifinal points in Madrid this week. Making his fourth appearance in Madrid last season, Cuevas registered four solid wins over the likes of Thomaz Bellucci, Nicolas Mahut, Benoit Paire and Alexander Zverev before losing to Dominic Thiem in the semifinal.
Though Cuevas is far from his best so far in 2018, he did reach the quarterfinal at Rio de Janeiro (clay) and the semifinal at Sao Paulo in February 2018.
